Tesamorelin is a synthetic peptide built from 44 amino acids and classified with the growth hormone–releasing hormone family. Its sequence corresponds to the human GHRH(1-44) backbone, carrying one structural change at the amino terminus. That change is a trans-3-hexenoyl group placed where the natural peptide would have an unmodified end. The modification is the feature that separates the compound from the endogenous hormone in name, in stability, and in how it is handled in the laboratory.
The hexenoyl cap slows the enzyme step that trims the amino terminus of native GHRH, the same step that shortens its active lifetime in circulation. As a result, the modified peptide persists longer in plasma than the unmodified hormone in side-by-side comparison. Receptor activity stays broadly comparable, because the added group sits away from the residues that contact the binding site. This combination, preserved receptor activity with reduced degradation, explains why the analog was developed instead of the native sequence.

Interest in this peptide developed because native GHRH has a short circulating lifetime. The N-terminal modification slows cleavage by dipeptidyl peptidase IV, an enzyme that removes the first two residues of many peptides and terminates their activity. Slower degradation means a longer window of receptor stimulation per administration. This design logic parallels other modified peptide hormones, where a small chemical change at a vulnerable site yields a more durable molecule without altering the core mechanism of action.
The peptide is synthesized chemically rather than extracted from biological sources. Solid-phase synthesis builds the chain from the C-terminus toward the N-terminus, after which the hexenoyl group is attached. Purity is typically assessed by high-performance liquid chromatography, and identity is confirmed by mass spectrometry. Regulatory review of the finished product focuses on these analytical controls, since small deviations in sequence or modification can change biological activity. Questions about long-term effects on the pituitary axis remain areas of continued investigation.
Tesamorelin is a synthetic peptide that belongs to the growth hormone-releasing hormone (GHRH) family. Its sequence corresponds to the fully active 44-amino-acid form of human GHRH, with a single structural modification: the addition of a trans-3-hexenoyl group at the N-terminus. That modification is not found in the naturally occurring hormone and was introduced deliberately during development to improve stability against enzymatic degradation. The compound is therefore best described as a stabilized analogue rather than a naturally occurring peptide.
Binding of tesamorelin to GHRH receptors on pituitary somatotroph cells triggers cyclic AMP signaling and the release of growth hormone into circulation. Because the peptide acts upstream of the growth hormone axis, its effects are partly mediated by hepatic insulin-like growth factor 1 (IGF-1) production. The pulsatile character of endogenous growth hormone secretion is preserved rather than replaced. Whether amplified signaling produces effects beyond those of native GHRH remains an area of ongoing investigation.
A documented effect of tesamorelin is a reduction in visceral adipose tissue in some study populations. Researchers have reported decreases in trunk fat measured by computed tomography alongside changes in lipid markers. The mechanism is thought to involve growth hormone-mediated lipolysis, though the precise contribution of direct versus indirect pathways is not fully resolved. Studies have generally examined defined groups over finite periods, so long-term outcomes are less well characterized. Findings have not been uniform across all trials.
=== Intrauterine administration === A one-year progesterone intrauterine device (IUD) for hormonal birth control was previously available in the United States and a few other countries under the brand name Progestasert. It was marketed between 1976 and 2001. The IUD was never widely used due to a relatively high contraceptive failure rate of 2.9% and the requirement of annual replacement. It contained 38 mg progesterone and released 65 μg progesterone into the uterus per day (totaling up to about 35 mg after one year). For comparison, a woman's body produces on average about 25 mg progesterone per day during the luteal phase. While effective as a form of contraception and for decreasing menstrual bleeding and discomfort, long-lived IUDs can fundamentally only deliver small amounts of progesterone per day, and hence intrauterine progesterone cannot achieve adequate circulating progesterone levels and is unsuitable as a form of systemic therapy. Aside from progesterone, IUDs of progestins, such as levonorgestrel (Mirena/Levosert/Skyla), are available as well.
=== 24 March === The RSF launched a series of attacks in eastern Khartoum, killing at least five people and injuring dozens. At the same time, the group retreated from the Salama, Azhari, Ad Hussein, Mayo, Gereif West, Burri, Sahafat and Kalakla neighbourhoods of Khartoum and were seen moving south towards Jebel Aulia. The Darfur Initiative for Justice and Peace said that hundreds of civilians were killed in an SAF airstrike on the Tur'rah market near El Fasher.
== Research == Stem cell therapy may represent a treatment for promoting the healing of diabetic foot ulcers. Diabetic foot ulcers develop their own, distinctive microbiota. Investigations into characterizing and identifying the phyla, genera, and species of nonpathogenic bacteria or other microorganisms populating these ulcers may help identify one group of microbiota that promotes healing. The recent advances in epigenetic modifications, with special focus on aberrant macrophage polarisation, are giving increasing evidence that epigenetic modifications might play a vital role in changing the treatment of diabetic foot ulcer in the near future.
=== Mechanism of action === Cetacaine acts quickly in about 30 seconds and can last between 30–60 minutes. This is due to benzocaine causing the immediate anesthetic effect, while butamben and tetracaine hydrochloride causes the extended effect of Cetacaine. The actual mechanism for the onset of anesthesia is unknown, but it is believed that the active ingredients reversibly block nerve conduction therefore causing the numbing sensation. This stabilizes the neuron and prevents signals from being transferred.

== History == Vosilasarm was developed by Radius Health in 2010. It was first described in the literature in 2011 in a paper detailing its design, synthesis, and preclinical characterization in vitro and in rats and monkeys. It was stated in this paper that phase 1 clinical studies of vosilasarm for treatment of severe weight loss due to cancer cachexia were being prepared. However, these studies were never completed or published and development for this indication was discontinued. Subsequently, vosilasarm was repurposed for the treatment of breast cancer.

No. It is a peptide that acts upstream of growth hormone release, while growth hormone is the hormone itself. The two differ in size, in receptor, and in how the body clears them.
It blocks the amino-terminal degradation step that limits native GHRH. The addition extends how long the peptide survives in plasma without removing its ability to activate the receptor.
The chain contains 44 amino acid residues. It matches the human GHRH(1-44) sequence apart from the amino-terminal modification.
tesamorelin 属 GHRH 类似物,作用于垂体受体以促进内源生长激素释放;生长激素本身是直接补充的外源激素。两者在给药逻辑、作用位点和反馈调控路径上并不相同。
