== Government action == The FDA filed a complaint for injunction in 2014, alleging that the company QLaser PMA was marketing its devices as being able to treat "over 200 different diseases and disorders," including cancer, cardiac arrest, deafness, diabetes, HIV/AIDS, macular degeneration, and venereal disease. This case resulted in a permanent injunction against the manufacture, marketing, sale, and distribution of those devices in 2015. In 2017, the owner of QLaser, Robert Lytle, and two of QLaser's distributors were charged with a criminal conspiracy to commit fraud. Lytle pleaded guilty to one count of conspiracy to introduce misbranded medical devices into interstate commerce with the intent to defraud and mislead, and one count of criminal contempt in January 2018. Lytle was sentenced to serve 12 years in prison and made an initial restitution payment of $637,000. Lytle's conspirators were sentenced to 24 months and 15 months, respectively.
== Medical uses == Elagolix is used in the treatment of moderate to severe pain associated with endometriosis in premenopausal women. Endometriosis is a condition in which the endometrium, the inner lining of the uterus, grows outside of the uterus into surrounding tissues and causes symptoms such as pelvic pain and infertility. Around 10% of women may be affected by endometriosis. Elagolix significantly decreases symptoms of dysmenorrhea (menstrual pelvic pain), non-menstrual pelvic pain, and dyspareunia (pain during sexual intercourse) in women with endometriosis. The medication is used at a lower dosage of 150 mg once per day or at a higher dosage of 200 mg twice per day, depending on the severity of symptoms. The effectiveness of elagolix in the treatment of symptoms of endometriosis was demonstrated in the 6-month Elaris Endometriosis I and II (EM-I and EM-II) phase III clinical trials. In Elaris EM-I, the percentage of women who had a clinical response with respect to dysmenorrhea was 46.4% in the lower-dose elagolix group and 75.8% in the higher-dose elagolix group, as compared with 19.6% in the placebo group; in Elaris EM-II, the corresponding percentages were 43.4% and 72.4%, as compared with 22.7% (P < 0.001 for all comparisons).
==== Multicenter ESG Randomized Trial (MERIT) ==== The recently published MERIT study is the first and only randomized controlled trial of the ESG for treatment of obesity and included 209 adult subjects with class I and II obesity (BMI 30–40 kg/m2). It was conducted from Dec. 2017 to Jun. 2019, spanned nine centers in the United States, and comprised 85 adults in the ESG arm and 124 adults in the control arm (lifestyle modification only). Subjects in the ESG arm lost 13.6% of body weight and 49.2% of excess body weight at one year, compared to 0.8% total body weight loss and 3.2% excess weight loss in the control arm at the same time point.

=== Obesity === It is well known that children, adolescents, or adults with OSA are often obese. People with obesity show an increase in neck fat tissue, which potentiates respiratory obstruction during sleep. However, people of all ages and sexes with normal body mass indices (BMIs) can also demonstrate OSA – and these people do not have significant measures of subdermal or intra-neck fat as shown on DEXA scans. It is speculated that they may have increased muscle mass or a tendency toward decreased muscle tone, potentiating airway collapse during sleep. However, loss of muscle tone is a key feature of deep sleep, and whilst obesity seems a common association, it is not an invariable state of OSA. Sleeping supine (on one's back) is also represented as a risk factor for OSA. Gravity and loss of tongue and throat tone as a person enters deep sleep are clear factors contributing to OSA development. But this explanation is also confounded by the presence of neck obesity. The use of CPAP primarily expands a collapsed upper airway, allowing for nasal breathing. A positive response to CPAP confirms that airway collapse is the cause of OSA. Throat lesions, particularly enlarged tonsils, are well recognized as aggravators of OSA, and removal may provide full, partial, or semi-permanent relief from OSA, which also indicates that enlarged tonsils may play a role in the pathogenesis of OSA.

== Career == Laidlaw attended the University of Oregon, where he tried, and was discouraged by, punched card computer programming. While working at the Pacific Gas and Electric Company, Laidlaw wrote the novel Dad's Nuke, which he followed with the novels 37th Mandala, Kalifornia and The Orchid Eater. He wrote a series of articles for Wired profiling the video game developer id Software. He was commissioned to write a novelization of the FMV adventure game Gadget by the Japanese developer Synergy. Gadget was unsuccessful, but Laidlaw was excited by the creative process of game development. While writing another id profile, he began asking about game design and exploring opportunities in the industry.

Measurement of total bilirubin includes both unconjugated (indirect) and conjugated (direct) bilirubin. Unconjugated bilirubin is a breakdown product of heme (a part of hemoglobin in red blood cells). The liver is responsible for clearing the blood of unconjugated bilirubin, by 'conjugating' it (modified to make it water-soluble) through an enzyme named UDP-glucuronyl-transferase. When the total bilirubin level exceeds 17 μmol/L, it indicates liver disease. When total bilirubin levels exceed 40 μmol/L, bilirubin deposition at the sclera, skin, and mucous membranes will give these areas a yellow colour, thus it is called jaundice. The increase in predominantly unconjugated bilirubin is due to overproduction, reduced hepatic uptake of the unconjugated bilirubin and reduced conjugation of bilirubin. Overproduction can be due to the reabsorption of a haematoma and ineffective erythropoiesis leading to increased red blood cell destruction. Gilbert's syndrome and Crigler–Najjar syndrome have defects in the UDP-glucuronyl-transferase enzyme, affecting bilirubin conjugation. The degree of rise in conjugated bilirubin is directly proportional to the degree of hepatocyte injury. Viral hepatitis can also cause the rise in conjugated bilirubin. In parenchymal liver disease and incomplete extrahepatic obstruction, the rise in conjugated bilirubin is less than the complete common bile duct obstruction due to malignant causes. Limited/selective fasciectomy removes the pathological tissue, and is a common approach. A 2015 Cochrane review reported that low-quality evidence suggested that fasciectomy may be more effective for people with advanced Dupuytren's contractures. During the procedure, the person is under regional or general anesthesia. A surgical tourniquet prevents blood flow to the limb. The skin is often opened with a zig-zag incision but straight incisions with or without Z-plasty are also described and may reduce damage to neurovascular bundles. All diseased cords and fascia are excised. The excision has to be very precise to spare the neurovascular bundles. Because not all the diseased tissue is visible macroscopically, complete excision is uncertain. A 20-year review of surgical complications associated with fasciectomy showed that major complications occurred in 15.7% of cases, including digital nerve injury (3.4%), digital artery injury (2%), infection (2.4%), hematoma (2.1%), and complex regional pain syndrome (5.5%), in addition to minor complications including painful flare reactions in 9.9% of cases and wound healing complications in 22.9% of cases. After the tissue is removed the incision is closed. In the case of a shortage of skin, the transverse part of the zig-zag incision is left open. Stitches are removed 10 days after surgery. After surgery, the hand is wrapped in a light compressive bandage for one week. Flexion and extension of the fingers can start as soon as the anaesthesia has resolved. It is common to experience tingling within the first week after surgery.
